CoolSculpting and other cryolipolysis devices.

These systems freeze fat to kill fat cells. Although many people have acceptable outcomes, a specific complication called paradoxical adipose hyperplasia can occur, where fat in the treated area grows rather than shrinks weeks to months later. This complication is recognized in the medical literature and in device labeling, and it often requires surgical correction.

Liposuction. 

Surgical fat removal can deliver dramatic reduction quickly, but it is still surgery. Complications include contour irregularity, seroma, infection, thromboembolic events, anesthesia risks, and in rare cases, mortality. Surgical risk is unavoidable because tissue is mechanically removed.

Bariatric surgery, including gastric bypass, sleeve gastrectomy, and adjustable gastric banding. 

These procedures can be life-saving for qualified candidates with severe obesity and metabolic disease. The latest weight-loss craze involves GLP-1 agonist drugs. These medications were developed for type 2 diabetes, and weight loss was first noticed as a side effect in that population. Manufacturers then capitalized on this observation by studying higher doses and launching versions branded for chronic weight management. In the pivotal STEP-1 obesity trial of semaglutide 2.4 mg, gastrointestinal side effects such as nausea and diarrhea were common, even as significant weight loss occurred. The risks are not hypothetical. 

A 2022 meta-analysis of 76 randomized trials found that GLP-1 drugs increase the risk of gallbladder and biliary disease, and the risk was higher in weight-loss trials and with higher doses. [DOI: 10.1001/jamainternmed.2022.2364] 

The FDA-approved prescribing information for Wegovy documents acute pancreatitis cases and higher rates of gallbladder events than placebo, which are warnings in official labeling that guide prescribing and monitoring—not an opinion. Ozempic labeling similarly notes postmarketing reports that include cholecystitis and cholecystectomy among hepatobiliary adverse events. Tirzepatide, another current agent, produced frequent gastrointestinal adverse events in the SURMOUNT-1 trial, with more discontinuations at higher doses. [DOI: 10.1056/NEJMoa2206038]
